LINKS is proud to work with the Veterans Administration to help facilitate the Mat-Su Veterans-Directed Care program. The Veterans-Directed Care program provides Veterans with opportunities to self-direct their personal care services, and to continue living independently at home for as long as possible. Veterans enrolled in this program manage their own flexible budgets, decide what mix of goods and services best meet their needs, and hire and supervise their own workers. The Veterans Directed Care Program Services Coordinator provides facilitated assessment and care/service planning, arranges fiscal management services, and provides ongoing counseling and support to veterans, their families, and caregivers.
The Veteran-Directed Care Program Services Coordinator (VDCP) works closely with Veterans who need in-home assistance to help connect them with the services and supports they need to maintain their independence within their home and community. The VDCP Coordinator will be expected to aid veterans, seniors, adults with disabilities and caregivers by providing unbiased options counseling and decision-support regarding available community and federal resources.
Specific job duties include:
Learning about community, state and federal resources available to eligible Veterans
Identifying appropriate resources and providing resource education to Veterans and their families
Assisting with resource referrals, applications and access to programs
Assistance with systems navigation and education to help Veterans maintain their benefits
Timely submission of all documentation
LINKS is a collaborative environment which offers multiple programs designed to assist Seniors, People experiencing disabilities of all ages and their caregivers. The VDCP Services Coordinator will be expected to perform other various duties and tasks as assigned to empower all individuals in the Mat-Su community to gain access to the information, assistance and resources they require.
This position works with the lead services coordinator and is supervised by the Aging and Disability Resource Center(ADRC) program director.
Full time, 40 hours a week
Hours: Daytime, 8-4 or 9-5 with very occasional evenings.
Location: Due to Covid-19 we are currently working from home. We are in the planning phases of returning to work from the LINKS office. Applicants should plan on working from home for the first month to 18 months of the position.
Pay: $18.00/hour DOE
Bachelor’s degree or professional equivalent: (4 years of experience specifically serving seniors and/or veterans)
Ability to learn and effectively explain specific human service delivery systems, social service resources as well as VA, state, and federal benefits programs

Provide person-centered assistance with a respect for client self-determination
Exercise good judgment and self-care
Maintain healthy professional boundaries
Ability to perform home visits including safely navigating a variety of home environments
Document and record all services provided in a timely manner

Ability to work independently and as a team in a community setting
Ability to work under pressure and meet tight deadlines
Desired Qualifications and Experience:
Due to the nature of this position, we have a Veteran hiring preference
Computer literacy/ability to be trained on specific systems and programs
Cultural competency and experience with diverse populations
Must be able to pass a background check
Current Alaska driver’s license and proof of insurance
